Administration of Medicines/General Care Plan

Administration of Medicines Form

 

If your child needs to take prescribed or non-prescribed medication whilst in school then a completed Administration of Medicine form must be completed and returned to the school office.

We also ask that parents complete a form if their child uses an inhaler for asthma and also adrenaline auto-injector for any allergy.
